Session 1: Project Well-Being – Together We Thrive with BCBS of MN, Mankato Clinic, WellShare International

Learn how Blue Cross Blue Shield of Minnesota, WellShare International and the Mankato Clinic, are implementing Project Well-Being at the Mankato Clinic. This video presentation is from the 2021 Rural Equity Summit. By reaching out to patients in their clinic, Project Well-Being identifies and connects patients to community health workers who help patients handle issues that impact their health and well-being – housing, language, work, food, transportation, insurance and loneliness. CHWs bridge the gap between the patient, healthcare team, insurance, social services and community resources.

Session 2: Introduction to MN Dept. of Agriculture Emerging Farmers Working Group

The Emerging Farmers Working Group works to identify and break down barriers which have historically prevented existing MDA programs from reaching underrepresented communities. This group advises the commissioner and legislature regarding the development and implementation of programs and initiatives that support emerging farmers in this state. The working group consists, to the extent possible, people and organizations that represent, farmers or aspiring farmers who are women, veterans, persons with disabilities, American Indian or Alaskan Natives, members of a community of color, young, and urban, and any other emerging farmers as determined by the commissioner. This video session from the 2021 Rural Equity Summit will provide an overview of their work.
